Job Description
Position/Job Title: Day Habilitation Services Provider
Department: Community Services
Essential Functions and Scope of Responsibilities:
The Day Habilitation Services Provider has the primary responsibility to provide community based support and assistance achieving goals related to social activities and life skills.
Job Duties:
Will complete note charting/documentation of clients progress legibly, accurately, and timely Must be able to accurately document and track progress of goals May supervise/assist client in tracking medications and health care needs Must have use of a reliable vehicle Must possess the ability to perform all tasks and activities as needed by this service, as well as exercise sound judgment and provide quality care Possess the ability to communicate effectively with the client, family, and your supervisor May be required to manage finances for client with proper documentation Must have the ability to work on an individual basis or closely with a team.
Day Habilitation services must be provided outside of the home. It is based in a community setting

Educational Requirements:
High School Diploma or GED
Qualifications and/or Experiences:
Must be a minimum of 18 years of age
Be free of communicable diseases and other conditions that would pose a substantial safety or health risk to clients Meet the Day Habilitation provider requirements as provided by Senior Disabilities Services and/ or The Department of Developmental Disabilities No convictions of abuse, neglect, or any other crime that would pose a threat or health risk to clients or their families Proof of Insurance Coverage
