Washington County Mental Health Services Case Manager in Barre, Vermont

POSITION:                Case Manager

PROGRAM:              School-Based Services

DIVISION :                 Children, Youth and Family Services

SUMMARY OF POSITION:   Develops and /or delivers ongoing community-based assessment, treatment and supports for children and youth experiencing a severe emotional disturbance and their families. Full time with benefits.

SPECIFIC DUTIES & RESPONSIBILITIES:

  • Develops and / or delivers services to assist clients in gaining access to needed medical, social, educational, vocational, housing, financial and other services. 

  • Maintain contact with other professional service providers for the purpose of case review or consultation regarding services to a specific client.

  • Develop treatment plans to deliver, or provide for the delivery of, services and interventionist identified by the treatment team.

  • Assist in developing natural support networks for children and youth experiencing a severe emotional disturbance and their families.

  • Assist children and youth experiencing a severe emotional disturbance and their families in accessing community resources.

  • Participate in ongoing treatment and educational team meetings.

  • Work cooperatively with schools, social rehabilitative services and other agencies providing services to children and youth experiencing a severe emotional disturbance and their families

  • Provide monthly case management and specialized Rehab. Summaries.

  • Attend staff meetings at WCMH as required.

  • Ability to physically restrain children and youth who are a danger to self or others.

  • Willing to learn crisis de-escalation and passive restraint techniques.

  • Problem solves and manage crisis.

  • Maintain on-going communication with school staff.

  • Responsible for supervising up to 9 Behavioral Interventionist staff .

  • Other duties as assigned by supervisor.

    QUALIFICATIONS:

    Education:   Bachelor’s Degree in human service or related field required or in process of completion.  Master’s Degree and / or enrolled in an Applied Behavioral Analysis program preferred.                       

    Experience:    Two years of human service delivery with children and families preferred.  Experience providing direct instruction and therapeutic services to children with challenging behaviors preferred.  Possession of a valid driver’s license, excellent driving record and access to a safe, reliable, insured vehicle are required. 

    Duties are performed in a combination of office settings and in the field. Incumbents must be able to:

  • Move from standing to sitting without accommodations,

  • Walk up to 1 mile in a continuous session,

  • Able to execute or willing to learn to execute physical restraints and able to lift and carry 50 pounds and execute physical restraints.

  • Having private means of transportation is required for necessary travel.

  • Engage in effective working relationships with a number of other professional disciplines.

    Certain functions may require duty outside of normal work hours. Some resistance or objection to standards may be anticipated, requiring tact and diplomacy to achieve proper action.
