Summary This position is located in the Social Work Service of the Bay Pines VA Healthcare System. Incumbent will provide services at C.W. Bill Young Medical Center. The duties and responsibilities are carried throughout the HealthCare Center including all clinical and other patient care areas involved with the services. Responsibilities Incumbent performs specialized treatment of complex physical or mental illness. Incumbent incorporates complex multiple causation in differential diagnosis and treatment of Veteran patients, including making psychosocial and psychiatric diagnoses within approved clinical privileges or scope of practice within the PACT clinic and Homeless Programs. Screening/Assessment In the area of screening and assessment, the Coordinator assists the Homeless PACT members in quickly triaging Veterans in the field, synthesizing available information along with the presenting problem, and distinguishing between brief and more in-depth responses. The social worker will develop a bio/psycho/social assessment of the Veteran in collaboration with the interdisciplinary treatment team, the Veteran, available family members and significant others. The incumbent must have an understanding of the problems of homelessness and respect for the diversity of the homeless Veteran population, being sensitive to human growth and development, age-appropriate needs and concerns of the homeless Veteran, and must be able to provide independent treatment of this Veteran population. The initial intake assessment will be completed at admission. Incumbent will input and retrieve information for the assessment from the Veteran, collateral sources with appropriate ROI's and CPRS, and will complete an assessment through the Homeless Operations Management Evaluation System (HOMES) and other data collection tools as required. Due to the transient nature of our identified homeless Veteran population, the social worker completes re assessments on a quarterly basis and sees the Veteran as often as needed to meet the goals established in the initial intake plan. A doctoral degree in social work may NOT be substituted for the master's degree in social work. Licensure: Persons hired or reassigned to social worker positions in the GS-0185 series in VHA must be licensed or certified by a state to independently practice social work at the master's degree level. Grade Determinations: In addition to meeting the basic requirements stated above, the following qualifications criteria must be met in determining the appropriate grade assignment of candidates. GS-12 Social Worker Program Coordinator candidates must have the following: Experience and Education: One (1) year of experience equivalent to the GS-11 grade level. Experience must demonstrate possession of advanced practice skills and judgment, demonstrating progressively more professional competency. Candidate may have certification or other post-master's degree training from a nationally recognized professional organization or university that includes a defined curriculum/course of study and internship, or equivalent supervised professional experience. Licensure/Certification: Individuals assigned as social worker program coordinator must be licensed or certified at the advanced practice level, and must be able to provide supervision for licensure. Skill in organizing work, setting priorities, meeting multiple deadlines, and evaluating assigned program area(s). Ability to provide training, orientation, and guidance within clinical practice. Physical Requirements: The physical demands of the work requires use of fingers; moderate walking and standing; near vision correctable at 13" to 16" to Jaeger 1 to 4; far vision correctable in on eye to 20/20 and to 20/40 in the other; hearing aid permitted; clear speech; emotional stability; moderate duty.
