Job/Position DescriptionClinical Coord PS IIIThe Clinical Coordinator performs complex (journey-level) consultative services and technical assistance work. Work involves planning, developing, and implementing an agency program and providing consultative services and technical assistance to program staff, government agencies, community organizations, or the general public. May provide guidance to others. Works under general supervision, with moderate latitude for the use of initiative and independent judgment.Essential Job FunctionsThe primary responsibilities for this position will be to intervene at the child and family level, adopting an interdisciplinary team approach, which includes primary stakeholders involved with the child including, a STAR Health Medical Case Manager as well as professionals from the courts, education, juvenile justice, mental health, family members and others, to quickly get children out of unlicensed care, prevent entry into unlicensed care, and decrease recidivism.Develop trauma-informed staffingand#146;s with the interdisciplinary team and provide trauma-informed, individualized plans of care created with youth and family.Work with each childand#146;s caseworker to ensure youth and their families are adequately prepared and supported to fully engage in these staffingand#146;s. The Clinical Coordinator should practice in alignment with the core principles and values so that family-based care is optimized and there is an individualized approach that does not subscribe to cookie cutter step down pathways. The Clinical Coordinator should work with the caseworker and the regional lead to ensure that every child in an unlicensed placement is receiving the necessary therapeutic and educational resources needed.The Clinical Coordinator will work with caseworkers to follow children for a minimum of 90 days after discharge from unlicensed placement to ensure that adequate services and supports are in place to stabilize them.As children are moved into licensed placements the clinical coordinators will transition to assist with facilitating discharge meetings/ discharges for foster children in acute, subacute and QRTP. The clinical coordinator will maintain consistent contact with the members of the childand#146;s team. This goal being to successfully discharge children from one placement to another through supporting the caseworker and child and leading the team throughout the discharge process. This position will work closely with other entities to best support the child in DFPS care. The coordinator will be responsible for providing discharge support to other DFPS field staff.May recommend and coordinate activities to produce a more effective program. May provide input on developing and/or revising policy and procedure manuals.
