Brockton Hospital Clinical Social Worker in Brockton, Massachusetts

Position Summary:
Under the direction of the Director of the Social Work Department, provides assessment, advocacy and discharge planning to patients and their families experiencing social and personal adjustment problems caused or complicated by illness or disability. Patients served may be newborn, pediatric, adult, and geriatric.

Responsibilities:
This individual will have responsibility to provide for and/or support the Interdisciplinary Team with the following:
Care Coordination of patients including assessment and discharge planning with psychosocial risk, including but not limited to domestic violence, protective services, frail elders, disabled, issues related to substance use and mental health, homelessness, hospice care and end of life issues, health care proxy and guardianship issues, maternal and pediatric care needs
Crisis Intervention
Mandated Reporting
Education and Advocacy of patients, families, and or significant others
Demonstrates respect and regard for the dignity of all patients, families, visitors, and fellow employees to ensure a professional, responsible, and courteous environment.

BASIC KNOWLEDGE/SKILLS/APTITUDE/EXPERIENCE:

Education/Experience/Licenses/Technical/Other:
Education: Master’s Degree in Social Work
Experience: 2-5 years of acute hospital experience preferred
Certification/Licensure: Licensed or License eligible, as a LCSW in the state of MA
Software/Hardware: Microsoft products and Meditech
