Family Service Association - Fall River Registered Nurse - Brockton in Brockton, Massachusetts

Family Service Association is looking for a caring Registered Nurse – Care Coordinator for our LTSS Community Partners (LTSS CP) program!  This a 35 hour work week, day time schedule.   Potential travel in the Greater Brockton region and occasional travel to Fall River.

The LTSS CP Registered Nurse-Care Coordinator;

  • Provides both telephonic and in-person consultation for acute changes of status in members

  • Review members’ medical history, medical needs, medications and functional status, including needs for assistance with any activities of daily living (ADLs) and instrumental activities of daily living (IADLs) as set forth in the comprehensive assessment

  • Assist with development of care plan goals to improve health outcomes

  • Conduct medication reviews and document such reviews in the member’s electronic health record (EHR)

  • Maintain all documentation required for the delivery of services including other data entry into the EHR, as required

  • Triage and balance competing priorities

  • Work independently as well as part of a multidisciplinary Care Team

    Education , License Requirements, and Experience:

  • Registered Nurse (RN) licensed in the Commonwealth of Massachusetts in good standing.

  • Experience working with youth and adults, individuals with complex medical needs, and disabled populations ages 3 - 64.
