Evergreen Health Behavioral Health Nurse Care Manager in Buffalo, New York

The Behavioral Health Nurse Care Manager provides services onsite, and collaborates with staff and outside agencies in relation to OASAS substance abuse services. They will work toward ensuring the quality and effectiveness of program activities are met. The Behavioral Health Nurse Care Manager works in collaboration with primary care staff, pharmacies and insurance companies in order to facilitate patient care. As part of the essential functions of this role, the Behavioral Health Nurse Care Manager:

  • Provides comprehensible, billable nursing assessments, groups and health monitoring follow up, including providing coverage in Mental Health and Low Threshold MAT programs as needed.

  • Schedules patients per protocols (monitors attendance records and toxicology results; summarizes/organizes data from multiple treatment providers; determines potential program track designation for new patients based on history; generates new and follow-up patient summaries for each prescriber encounter).

  • Streamlines communication between prescribers and internal staff.

  • Coordinates with pharmacy and insurance companies to ensure coverage for patient prescription medications. Completes prior authorization paperwork when required.

  • Facilitates and leads collaborative multidisciplinary treatment team meetings.

  • Develop and maintain referral/resource list, contacts, and other pertinent clinical/treatment information.

  • Facilitates and leads collaborative multidisciplinary treatment team meetings.

  • Provides brief counseling for substance users regarding general health and substance use that are billable.

  • Provides comprehensible, billable nursing assessments, groups and health monitoring follow up .

  • Monitors hospital discharges, provides treatment team updates and 24 hours follow up with patients.

  • Orders supplies as needed; researching and recommending upgrades to equipment.

  • Works across both Substance Use and Mental Health Departments .

  • Provides trainings and educational services to non-medical treatment team staff.

  • Provides clinical trainings for newly hired staff.

  • Ensure adherence to all required OASAS Complex Care Management regulations, activities, and follow proper billing and documentation protocols.

  • Works within an integrated treatment model coordinating treatment with both internal and external providers as indicated.

  • Adheres to Office of Mental Health (OMH) and/or Office of Addiction Services and Supports (OASAS) regulations and Evergreen Health policy and procedures including regulatory timeframes on clinical documentation using an electronic medical record system.

  • Collaborates and support the team members to ensure a patient-focused approach to care.

    Qualified Candidate will have a current license to practice as a registered nurse in New York State as well as a current BCLS certification. The qualified candidate will have a minimum of two (2) years of behavioral health nursing experience, medication-assistant treatment clinic preferred. Candidate will have a familiarity with electronic charting systems (Medent preferred). Candidate must be able to communicate clearly and professionally, and demonstrate personal responsibility and integrity. Ability to handle Protected Health Information (PHI) in a manner consistent with HIPAA a must. Sensitivity to and experience with cultural diversity, Harm Reduction, LGBTQ population, Substance Use population and people living with HIV/AIDS issues is essential.

    Job Type: Full-time

    Required Certification: License to practice as a Registered Nurse in NYS

    Required experience: Two (2) years of behavioral health nursing (Medication-assisted treatment clinic preferred).

    Additional requirements: Current BCLS certification
