UC HEALTH LLC Social Worker(MSW), Part-Time, First Shift, Care Management Services in CINCINNATI, Ohio

Location: UC Medical Center Department: Care Management Services Hours: Part-Time, 20 Hours/Week Shift: First, 8:30am-5:00pm (Every 10th weekend and 1 holiday a year) Social Worker, Part-Time, First Shift, Care Management Services UC Health is hiring a part-time Social Worker for the Care Management Services department for the first shift at UC Medical Center. Social Workers serving in a variety of departments and clinics are responsible for responding to referrals from the interdisciplinary team. The social worker will complete psychosocial assessments, develop and implement a plan of care that addresses patients' needs that promotes continuity of care, and facilitates transitions to the next level of care. The social worker will provide education, consultation, and serve as a liaison to the healthcare team regarding the patient and families' emotional, psychological, social, financial and developmental aspects of care.

  • Education: Master of Social Work from an accredited School of Social Work, minimum required.
  • License and Certification: Licensed by the State of Ohio Social Work Board (LSW, LISW, or LISW-S)
  • Years of experience: One-two Years equivalent experience, minimum required. Three-five years, preferred.
  • Required Skills and Knowledge: Casework principles, including diagnosis, assessment, crisis intervention, and treatment planning; techniques in individual, family and group therapy; awareness of community resources, public assistance systems, Medicare/Medicaid; accepted professional and ethical standards within the existing medical community and professional organizations.
