Whidbey General Hospital Hospice Medical Social Worker in Coupeville, Washington
POSITION SUMMARY The Hospice Medical Social Worker, in collaboration with the interdisciplinary team (IDT), provides and facilitates attainment of economic, social, and emotional support services to hospice patients and their families/caregivers. Principle functions include but are not limited to, assisting the patient and family/caregiver with placement. The MSW will assess patient/family/caregiver social, emotional, and financial factors in order to cope with the terminal illness and death and assess the caregiver’s ability to function adequately. In addition, an assessment of special needs related to cultural diversity including communication, space, role of family members and special traditions will be required. The MSW will assess and develop instructions and plan of care reflecting needs identified for homemaker assistance after identifying the developmental level of patient/family/caregiver and obstacles to the learning in the care of the patient. This role will act as a resource for staff members and as the Hospice’s Medical Social Worker representative in the community and helps to develop referral sources. EDUCATION AND EXPERIENCE
Master’s Degree in Social Work (MSW) from a school of social work accredited by the Council of Social Work Education required. Minimum of one (1) year paid social work experience required. Hospice experience preferred. Licensure in the State of Washington. Current BLS HCP verification. Maintain a valid WA State Driver’s License, personal insurance, and is insurable with the District’s Insurance Carrier.
