Medical Social Worker

Location: Coupeville, WA - 98239 (Whidbey Island)

Employment Type: Direct-Hire, Permanent

Setting: Health System

Pay: $32-$65

Shift: Full-Time or Per Diem

Qualifications:

  • WA LASW, LSWAA, LCWAIC, or LICSW (preferred) license

  • Master’s Degree in Social Work

  • 1 year experience as a Medical Social Worker

  • BLS

Key Duties:

  • Collaborates with the interdisciplinary team to provide economic, social, and emotional support services to hospice patients and their families.

  • Supports patients and families during critical transitions of care, ensuring they receive appropriate guidance and resources.

  • Conducts intake assessments to evaluate the needs of patients and develop tailored support plans.

  • Offers coaching and counseling to assist patients and families in coping with challenges related to hospice care.

  • Ensures quality assurance through thorough documentation and access to social service resources.
