Description

Summary:
Live-in caregiver, providing assistance to clients of Nuvance Health Caregivers at Home(NHCH), with activities of daily living (ADLs) and instrumental activities of daily living (IADLs) as directed on clients’ individual care plans, within the guidelines of the certified nursing assistant (CNA) scope of practice and according to the NHCH policies and procedures.
Responsibilities:
Assist the client with personal care activities including bathing, oral hygiene, feeding and dressing
Assist the client with exercises, ambulation, and transfer activities
Utilize knowledge in use of devices such as canes, walkers, crutches, and lifts
Independently assists the client with meal preparation/cooking and/or feeding.
Must have working knowledge of special diets, diet restrictions, and techniques to maintain sanitary kitchen conditions to prevent food-borne illnesses
Assist the client with medication prompts to remind the client to self-administer his/her medications
Identify and report changes in client condition
Provide basic homemaking and laundry assistance for client in the home
Follow policy and procedure for assigned client care activities
Maintain a safe and orderly work environment in the clients’ homes
Maintain professional boundaries and integrity of client financial and personal information in clients’ homes
Follow network/agency infection and prevention policies and procedures in clients’ homes
Provide companionship and engages in activities of interest to the client
Assist the client with light pet care as needed, such as walking a dog, feeding a dog/cat, changing a cat litter pan
Transport the client to appointments and errands
Requirements:
Graduate of a nurse aide, home health aide or patient care tech program or a minimum of 5 years’ experience working as an aide in home care
Desired: Valid Driver’s License, Satisfactory Driving Record (Meets requirements of NHCH policy)
Ability to read, write, speak and comprehend the English Language
Certified Nursing Assistant (Preferred)
HS Graduate or Equivalent
