Specific Responsibilities:

This is a Health Services Representative position working in the Delray Beach Health Center. Work is performed under the general supervision of the Senior Community Health Nursing Supervisor. This position requires independent judgment, initiative, applying general knowledge of business practices and skills in applying instructions to accomplish various job functions. This is a bilingual position fluent both in English/ Spanish or English / Creole. This position will function as a “Floater” between Health Centers providing services as needed.

This position is designated as a sensitive position and is required to maintain confidential information in accordance with the Florida Department of Health (FDOH) Palm Beach County Health Department (CHD) Information Security Policy, Protocol and Procedures. Confidential data sets include, clinical data sets such as patient information, epidemiological information. This position may view, update and release patient information using the Health Management System (HMS}, to view, update, and release information. This also includes access to Employee Activity Reports (EARs), Electronic Health Records (EHR), Surveillance Tools & Reporting System (STARS).

Assist in performing intake vital signs procedures for patients attending clinics. Completes the required maternity forms. Accurately documents findings. Notifies nurse and/or provider if results are abnormal. Assist physician and other providers in the examination of patients. Provides patient education. Instruct patients in proper technique of collection of specimens.

Prepares, packages and sends specimens to the laboratory. Checks, cleans, and restocks examination rooms to ensure they are ready for start of clinic each day. Empties biohazard bags as needed, cleans and autoclaves gynecological instruments. Ensures adequate inventory for clinic. Tracks and follows up patients with positive Quantitative Fluorescence Technique (QFT). Follow up patients with positive QFT to ensure they have clinic appointments. Notifying patients of future appointments for X-rays or treatment.

Assist in preparation of records for clinics, laboratory and provider consultation. Assist in orientation of new employees. Tracks clients lost to care and follow up with missed appointments and sending out letters. Calls patients to remind them of clinic appointments. Participate in patient health education.

Follows up with Latent TB Infection (LTBI) tracking of positive IGRA results.

Participate in regularly scheduled clinic team meetings, and Quality Improvement (QI) activities.

Perform other related duties as assigned.

Knowledge, skills and abilities, including utilization of equipment, required for the position:

  • Knowledge of basic medical and pharmaceutical terminology.

  • Knowledge of basic computer software and electronic health records.

  • Knowledge of instrument decontamination and sterilization.

  • Skilled in using medical instruments or testing equipment.

  • Ability to interview patients and obtain a complete medical history.

  • Ability to use computer, computer programs, and office machines.

  • Ability to establish and maintain effective working relationships, customer service skills.

  • Ability to communicate effectively to convey information (verbally and writing) to others.

  • Ability to plan, organize and coordinate work assignments.

  • Must be fluent both verbally and in writing in English/Spanish or English/Creole.
