RN Case Manager - Emergency Department

Work Schedule: Three 12-hour shifts (7am to 7pm)

Location: Detroit

  • Smooth Transitions:Facilitate timely and safe discharge plans, helping patients transition seamlessly to the next level of care.

We encourage you to apply if you:

EDUCATION/EXPERIENCE REQUIRED:

· Have graduated from an accredited BSN program.

· Possess RN licensure in the state of Michigan.

· Possess a minimum of three (3) years of acute care/inpatient nursing experience.

· Inpatient Case Management and/or discharge planning and care coordination experience strongly preferred.

· Can communicate effectively with patients, families, and hospital personnel, have strong critical thinking and creative problem solving skills and excel at planning.

· Are self-motivated and enjoy working in a fast-paced environment.
