Moore County Hospital District Clinic RN Case Manager in Dumas, Texas

This is a Full-time position located at the Moore County Family Health Clinic working closely with Dr. Corbin and his team. The hours for the position will be 8AM-5PM Mon-Fri. SUMMARY: Clinic Case Manager is responsible for coordinating and providing comprehensive care management services to patients within a rural health clinic setting. The Case Manager works collaboratively with healthcare providers, patients, and community resources to ensure patients receive timely, appropriate, and effective healthcare services. This role is critical in improving patient outcomes, enhancing patient satisfaction, and reducing healthcare costs through effective care coordination and resource management. Key Responsibilities: * Care Coordination: * Develop and implement individualized care plans for patients based on their specific health needs, preferences, and goals. * Coordinate with healthcare providers, specialists, and community resources to facilitate comprehensive care. * Monitor patient progress and adjust care plans as necessary to ensure optimal health outcomes. * Patient Advocacy: * Serve as a liaison between patients, families, healthcare providers, and community resources. * Advocate for patient needs and preferences within the healthcare system. * Educate patients and families about health conditions, treatment options, and available resources. * Resource Management: * Identify and connect patients with appropriate community resources and services, such as financial assistance programs, transportation, and social services. * Maintain an up-to-date directory of local resources and services available to patients. * Documentation and Reporting: * Maintain accurate and timely documentation of all patient interactions, care plans, and progress notes in the electronic health record (EHR) system. * Generate reports and analyze data to monitor patient outcomes and identify areas for improvement. * Compliance and Quality Improvement: * Ensure compliance with all federal, state, and local regulations, as well as clinic policies and procedures. * Participate in quality improvement initiatives to enhance patient care and clinic operations. * Patient Education: * Provide education to patients and families on managing chronic conditions, medication adherence, and healthy lifestyle choices. * Develop and distribute educational materials and resources to support patient self-management. * Team Collaboration: * Work collaboratively with the healthcare team, including physicians, nurses, and other healthcare professionals, to deliver coordinated and comprehensive care. * Participate in team meetings, case conferences, and training sessions.  EDUCATIONAL/EXPERIENCE REQUIREMENTS:  Registered Nurse with minimum of 2 years of experience in case management, care coordination, or a related role in a healthcare setting. Bachelor’s degree in nursing. LICENSURE / CERTIFICATION REQUIREMENTS: Current Texas RN license or RN compact licensure. Case Management Certification (CCM) or eligibility to obtain certification within one year of hire is preferred. Current CPR OTHER REQUIREMENTS: Strong organizational and time management skills. Excellent communication and interpersonal skills. Ability to work independently and as part of a team. Proficiency in using EHR systems and other healthcare technology. Knowledge of community resources and services available in rural areas. WORKING CONDITIONS AND PHYSICAL DEMANDS: The position may require travel to patients’ homes, community organizations, and other healthcare facilities. May involve working outside regular business hours to meet patient needs.
