Burcham Hills Retirement Community Clinical Liaison-RN in East Lansing, Michigan

We are looking for a

Full Time Clinical Liaison, RN Preferred. However, will consider applicants with significant clinical experience requiring comparable knowledge, skills and abilities. Must be available to work some weekends on-call.

Responsibilities include:

  • Assists with the planning and execution of Burcham Hills sales and occupancy levels in accordance with the marketing and sales plan.
  • Develop and maintain hospital relationships to ensure a smooth transition of care from the Home or Hospital to Burcham Hills.
  • Works as a secondary resource to the admissions department, in the absence of the admissions coordinates and is able to proficiently function in EMR and all admission duties.
  • Reviewing physicians' orders and gathering clinical information to evaluate a patient's care needs. Interacts with a variety of hospital staff from different departments to ascertain clinical information relevant to the prospective client.
  • Conducting assessments and review of clinical records to establish suitability of prospective admissions, and the patient's eligibility for care.
  • Understands basic pharmaceuticals, test results, therapy evaluations, pre-existing conditions, and possess a general medical knowledge as it pertains to Patient Driven Payment Model (PDPM).
  • Completes detailed Pre-Admission Screens, according to company policies and procedures and payer requirements.
  • Demonstrates thorough knowledge of core therapy referrals, and qualifying criteria by payer source.
  • Interacts clearly and professionally with patients and families, while gathering additional clinical information and past history.
  • Collaborates with Admission Manager, Care Managers, Admission Nurses, referral sources, and Burcham Hills staff to ensure that each client has a smooth transition. Communicates/coordinates bed offer activities for new client referrals with Burcham Hills staff and referral sources.
  • Recognizes opportunities for expanding hospital relationships and clinical service lines. Participates in Clinical Program Development as identified through market demand.
  • Educates physicians, discharge planners, and other acute care partner representatives about our quality rating, clinical programs, clinical competencies, and industry regulations to facilitate broader collaboration and insight.
  • Consistent communication with discharge planners and referral sources to drive census growth.
  • Frequent travel for face-to-face marketing and assessment of potential residents.
  • Consistent phone contact with referral sources to keep Burcham Hills "top of mind".
  • Acts as a liaison for potential residents and health care team.
  • Responsible for marketing all areas of The Center for Health and Rehab at Burcham Hills and Networking with local community groups, facilities, and businesses.

Assist in developing preliminary and comprehensive social service needs assessments and plan of care for residents to include identifying resident/client issues, needs and goals.

Encourage residents/clients and family members to participate in the development and review of care.

Interview residents and families to obtain social history.

Evaluate social and family information to assist in determining plans for social treatment.

Interpret and relay the social, psychological, and emotional needs of the resident/client and family to the interdisciplinary team.

Provide consultation to members of the community in efforts to solve the needs and problems of the resident through the development of social service programs.
