This position is full-time (40 hours/week) Monday - Friday. Employees are required to work our normal business hours of 9:00am – 6:00pm MST. It may be necessary, given the business need, to work occasional overtime or holidays when requested by client. Employees will be required to work some days in the field and some days from home.

We offer 3 – 4 days of paid training. The hours during training will be 9:00am to 6:00pm, Monday - Friday.  * Training will be conducted virtually from your home.*

Primary Responsibilities:

  • Manage assigned member caseload telephonically

  • Complete home engagement visits, when telephonic outreach attempts (3), are unsuccessful.

  • Create a positive experience and relationship with the member

  • Keep the member out of the hospital by supporting regular visits to their primary physician

  • Keep member actively engaged with their primary physician

  • Support the member to ensure adherence to medication plan

  • Proactively engage the member to manage their care

  • Provide member education

  • Help to keep members compliant with their care plans

  • Partner with care team (community, providers, internal staff)

  • Follows specific timeframes for communication, screening and facilitating of care team meetings with the member and the caregivers, and other members of the health care team to facilitate and coordinate the members approved plan of care.

  • Documents all information gathered for the initial assessment, development of the care plan using SMART goals, implementation, monitoring, and evaluating outcomes.

Required Qualifications:

  • High School Diploma / GED and/or equivalent experience

  • Experience with outbound calls to members, Physician’s, and/or Providers

  • Microsoft Word (ability to create, copy, edit, send & save documents)

  • Microsoft Excel (ability to create, copy, edit, send & save spreadsheets)

  • Microsoft Outlook (ability to create, copy, edit, send & save correspondence)

  • Must be able to navigate a PC to open applications, send emails, and conduct data entry.

  • Bilingual fluency in English and Spanish

  • Reside in El Paso, TX

  • Required to have a dedicated work area established that is separated from other living areas and provides information privacy.

  • Ability to keep all company sensitive documents secure (if applicable)

  • Must live in a location that can receive a UnitedHealth Group approved high-speed internet connection or leverage an existing high-speed internet service.

  • Ability to travel up to 25% of the time.

  • Must be 18 years of age OR older.

  • Ability to work full-time, Monday - Friday between 9:00am – 6:00pm MST including the flexibility to work occasional overtime given the business need

Preferred Qualifications:

  • Experience working in Managed Care, Behavioral Health, and Substance Abuse

  • Knowledge of TX Medicaid

  • Familiarity with the resources available within the community

  • Experience in women’s and or pediatric health care setting

  • Ability to obtain a Community Health Worker Certification within 6 months of being hired or a current Community Health Worker Certification.
