    Purpose :

    The person in this position will provide direct casework and counseling services to the patient/family as directed by the plan of care. The social worker provides psychosocial assessments and develops a corresponding care plan as coordinated with the Interdisciplinary Team

    Job Description :

  • Provide psychosocial assessments, direct casework, and counseling services to patients and their families.

  • Participate in the preparation, evaluation, and execution of the interdisciplinary care plan  to assure that the aims of continuity and comprehensiveness of care are accomplished.

  • Work collaboratively as a member of the Interdisciplinary Team(s).

  • Prepare clinical and progress notes for the clinical record per Hospice, state, and federal guidelines/standards.

  • Prepare documentation for the clinical record per Hospice, state, and federal guidelines and standards.

  • Maintain and update knowledge of community resources. Facilitate referral of patients/families to those resources as needed and desired.

  • Participate in taking on-call responsibilities as necessary including weekends and evenings.

  • Participate in the Interdisciplinary Team meetings, staff meetings, and other meetings as requested.

    Qualifications :

  • A Master’s degree from an accredited educational institution of social work is preferred or another related clinical degree

  • Must be a licensed social worker

  • Strong interpersonal and communication skills are required

  • Experience in health care and clinical social work is preferred

  • Knowledge of the process of death, dying and bereavement is required

  • Understanding of psychosocial factors operative in terminally ill patients and families required

  • Unlimited access to a reliable car

  • Current state driver’s license and auto insurance
