Fairview Health Services Team Coordinator in Elk River, Minnesota

Overview

We Fairview Health Services are looking for a dedicated Team Coordinator to join the phenomenal team at our Elk River clinic. This role is approved for 56 hours every two weeks, and is benefit eligible. The Fairview Clinic - Elk River has 21 specialties and over 35 different providers! The staff at this location have a positive relationship, and we see this walking down the halls. This is a more rural site which allows for relationships to be built with patients as they come in to see you over the years. Responsibilities Job Description

As an integral member of the primary care team, the team coordinator (TC) are crucial to overall quality of care and patient experience via phone calls and electronic messaging. Team Coordinators perform high-level functions to support the needs of the healthcare team and patients/families between office visits to ensure patient needs are effectively addressed. They will deliver direct support to patients on the coordination of medical information requests and updates, as well as referrals and appointment scheduling. The TC will use a variety of communication tools and systems in their work to support patients and their care team. Additionally, we ask the TC's to maintain complete, timely and accurate documentation for each patient interaction. They will also continually look for opportunities we can improve processes and workflow.

  • Performs telephone & MyChart message support & follow-up communication in a timely and professional fashion; uses communication systems for both routine & emergency transactions

  • Schedules, reschedules & maintains patient appointments & supports clinic leadership in schedule management

  • Prioritizes multiple tasks, interruptions, and changes

  • Find opportunities for improvement in workflow & participate in clinic and system meetings

Qualifications

Required

  • Successful completion of formal medical training program such as MA, HUC, NA or Medical Secretary, or min. of 1 year comparable experience in a healthcare setting

Preferred

  • 2 or more years’ experience working in a medical setting
