Foothills Gateway, Inc. Children's Case Manager in Fort Collins, Colorado
The Children's Case Manager is responsible for planning, coordinating, and monitoring services and supports provided to families of children receiving Family Support Services and/or Children's Medicaid waivers. The Case Manager will have an assigned caseload of individuals. The Case Manager will be responsible to schedule and facilitate initial assessments for eligibility for long-term care, Interdisciplinary Team meetings to develop and, as needed modify individualized plans, monitor the provision of services, supports and recommendations identified in Service Plans, assist in obtaining identified services and supports; maintain a master record and all necessary documentation; serve as the primary contact for obtaining and disseminating information; and facilitate and mediate conflict resolution. Inherent in all these functions shall be activities supporting families' rights, support, empowerment, and advocacy. Spanish bilingual preferred with a $1.00 increase in pay rate.
Requirements: Bachelor's degree required; or five years of relevant experience in the field of Long-Term Services & Supports, which includes Developmental Disabilities or some combination of education and relevant experience appropriate to the requirements of the position. Driving is a requirement for this position.
