Trinity Health Stroke Coordinator - Neuroscience Institute in Ft. Lauderdale, Florida

Description:

  • Coordinates and oversees the Comprehensive Stroke Center for Holy Cross Hospital, working collaboratively with others to provide excellent, evidence-based stroke care, partnering with the Neuroscience Advanced Registered Nurse Practitioner.

  • Co-chairs the Stroke Steering Committee collaboratively with the Stroke Medical Director; prepares Stroke Steering Committee agendas with input of the Neuroscience ARNP, the Medical Director and others; Documents and disseminates minutes

  • Develops, reviews, and updates standards of care, policies, clinical pathways, protocol order sets, and educational materials adapted from published best practice guidelines in partnership with nursing leadership, and Medical Directors of the Stroke Program.

  • With assistance and input from the Neuroscience ARNP and others, coordinates a survey site visit of the Joint Commission to maintain successful re-certification of the Primary Stroke Center through data collection, education and application submission.

  • Facilitates the education of staff, physicians, leadership and the community about stroke measures, evidence and prevention.

  • Educates nurses and other personnel as needed on the Standards of Care/Best Practice, documentation tools, patient education and other topics related to the care of the Stroke patient.

  • Oversees the Patient Education for the Stroke patients and updates educational materials.

  • Contributes to the Stroke Center orientation and continuing education as appropriate.

  • Identifies continuing education needs of nursing personnel, implements programs and activities to meet these needs.

  • Assists in development, implementation and assessment of staff competencies in the care of the stroke patients.

  • Oversees annual stroke education requirements are met annually by hospital and medical

  • staff.

  • Develops educational programs and materials for patients and families.

  • Reinforces need for lifestyle modification to patients and families after discharge from hospital and maintains a continuum of care.

  • Participates in community and educational outreach activities.

  • Maintains the Stroke Center Database & Monitors Patient Satisfaction

  • Maintains database using Get with the Guidelines software, for all stroke patient information, as required; trends data outliers for improvement opportunities.

  • Maintains Stroke Patient Satisfaction Surveys Process.

  • Provides accurate and timely clinical data and identifies opportunities for improvement.

  • Follows the PDSA model for improving performance as they relate to Stroke Measures and analyzes data abstracted for trends, patterns and performance levels that suggests opportunities for improvement.

  • Serves as a liaison for the stroke patient population in the Emergency Department, and responds as a member of the Acute Stroke Response Team.

  • Completes regular rounds in ED and on patient units to ensure continuity of care and compliance with the Stroke Program measures.

  • Acts as an expert resource, clinical practitioner, and role model in caring for and assisting with planning, implementing and evaluating care of Stroke patients.

  • Coordinates efforts for improvement, development and education with the Nurse Managers of the Stroke Units.

  • Works collaboratively with Physicians, Nursing, Rehab Medicine, Radiology, and Laboratory to assure timely, quality care.

  • Stays abreast of current research, attends educational seminars and obtains contact hours specific to the care of the stroke patients.

  • Participates in shared decision making, shared governance councils and hospital / nursing committees as assigned.

  • Other duties as assigned

Minimum Qualifications:

BSN required with Specialty Certification preferred

Licensure:

Current Florida RN License
