The opioid crisis has reached alarming proportions, with devastating consequences for individuals, families, and communities. Medication for opioid use disorder (MOUD) has emerged as a vital treatment approach, offering hope for recovery and reducing the harmful effects of Opioid Use Disorder (OUD). The CHEMICAL HEALTH Social Worker - MEDICATION FOR OPIOID USE DISORDER (MOUD) is responsible for comprehensive assessment, treatment coordination and implementation of effective MOUD interventions for Dakota County s MOUD program located within the Dakota County Law Enforcement Center. This position will be cross trained to provide assessment and coordination services to clients working with our crisis continuum, including the embedded social work program. This position involves treatment coordination and collaboration with clients, detox, hospitals, and community resources. Duties & Responsibilities These examples do not include all possible tasks in this work and do not limit the assignment of related tasks in any position of this classification. Regular attendance according to the position's management approved work schedule is required for all positions. Assist individuals in gathering necessary verifications to determine eligibility for the Behavioral Health Fund. Complete comprehensive assessments in custody and the community. Assess an individual's current need for treatment and assist them with appropriate treatment program recommendations and referrals. Consult with individuals, families of substance dependent persons, as well as other agency staff and professionals in the community. Provides treatment coordination services for clients including but not limited to; meeting with individuals at their corresponding treatment programs, coordinating a continuing care plan as they transition out of current treatment settings, assist with accessing resources and coordinating services in the community. Assess & Evaluate individual strengths and needs including safety and risk to develop a least restrictive treatment plan. Develop and implement case planning, evaluate document and monitor client progress toward goals. Assess Mental Health & Chemical Health needs, collaborate & coordinate with other community services provides needs and level of services. Provide Prenatal assessment and treatment coordination to pregnant women with an identified opioid use disorder (OUD); assist them in gaining access to needed medical, social, and education on prenatal issues & fetal development. Promote the health of recipients and foster independent compliance with health care services. Demonstrates a commitment to support cultural diversity County wide, actively supports the County's inclusion, diversity and equity initiatives, and works to address the needs of diverse and underserved communities. Maintain and apply knowledge of relevant policies, procedures, and statutes and professional practice. Complete agency, state, and federal required documentation as elated to client activity according to mandated timelines. May provide support to the adult services intake, mental health assessment, crisis continuum including embedded social work program, prepetition screening and vulnerable adult/adult protection program areas including being cross-trained to provide assessment for Rule 79 mental health targeted case management and conduct vulnerable adult investigations. ESSENTIAL JOB FUNCTIONS: Duties 1-11 are essential functions. Knowledge, Skills & Abilities and Work Environment Knowledge of ASAM criteria. Knowledge of chemical health, human development, family dynamics and systems, including crisis management and cultural humility. Knowledge of community resources and funding sources. Knowledge of medical diagnoses, medications and basic medical terminology, as well as medical insurance processes and procedures. Knowledge of statues, rules and policies pertaining to social work, substance use disorder treatment and the ability to research the same. Comprehensive knowledge of computer software and information systems. Interviewing, negotiation and listening skills. Ability to communicate effectively both orally and in writing. Demonstrates an understanding of and respect for the diversity of customers, co-workers, and supervisors, including individuals with a disability or whose first language may be other than English. WORK ENVIRONMENT: Work is performed in a hybrid environment including work from a County office building, on site visits to client's homes, hospitals, correctional facilities and withdrawal management facilities. Some travel is required. Lifting requirement of up to 20 pounds on an occasional basis. Exposure to intense noise and inadequate lighting may occur on visits to client's homes. Equipment used may include, but is not limited to, computer, printer, telephone, calculator, and copy and fax machines.
