What you will work: .8 FTE = 64 hours every 2 weeks Monday - Friday 7am - 4 pm What you will do: Schedule patients and manage provider schedules Interface with external agencies Ensure insurance and regulatory compliance Coordinate and communicati with team members to ensure best patient care What you will get: A top-rated retirement plan and healthcare benefits and much more A rewarding career A great team atmosphere What you need: High School diploma or equivalency 1 year office setting experience requiring customer service responsibilities computer competency in Microsoft Windows and Excel programs Job Description: The Transitional Services Coordinator coordinates the day-to-day operations of the Transitional Services pro-grams including: scheduling of patients, managing provider schedules, interfacing with external agencies, ensuring insurance and regulatory compliance, and coordinating and communicating with team members to ensure best patient care. This role is applicable to the following Behavioral Health programs: Residential Treatment, Partial Hospitalization, and Mental Health Day Treatment. This position reports to the Clinical Manager or Clinic Site Manager of the program. Major Responsibilities: 1. Represents GundersenHealth System\'shigh standards for customer service in all telephone and face-to-face encounters with both internal and external patients, family members, guardians, staff, customers, and physicians. 2. Facilitate department operations through knowing and communicating information, managing desk area, technology support for staff, monitoring and communicating intake/discharge status, and responsible for coordinating all day to day staff group coverage for the transitional department. Including coordination of patients\' appointments to meet regulation requirements of programming. 3. Demonstrates effective, collaborative working relationships with the health care team members. 4. Coordinates daily schedule for Residential, Partial Hospitalization, or the Mental Health Day Treatment programs, ensuring minimum number of programming hours are offered to meet DHS and/or Medicare billing requirements. 5. Assists throughout the entire registration, admission and discharge process as directed. 6. Participates in quality improvement activities in order to continuously improve patient care and department operations. 7. Attends and participates in staff meetings as directed. 8.
