Greater Lawrence Community Action Council Case Manager Specialist in LAWRENCE, Massachusetts

Job Summary:

The Case Management Specialist will directly work with DCF families to ensure results in case coordination of care and services for all assigned families, thereby achieving established targets and goals. This position, under the guidance of the case manager supervisor, requires collaboration and communication with families, DCF staff, as well as outside agencies and resources to enhance the quality and efficiency of service delivery and promote the health and well-being of CFSC’s families.

Essential Functions/Key Responsibilities:

  • Accepts referrals from the DCF. Manages a case load of 14 families.

  • Completes client intake and evaluations for the identified families. 

  • Face-to-face contact will occur a minimum of 1 hour per week or as determined by family needs.  

  • Performs required and supportive tasks related to the needs of the families. 

  • Develops and uses network of available services to assist families. 

  • Provides specific information concerning agency service procedures, determines client eligibility for services and other necessary information. 

  • Carries out case outreach and case management. 

  • Maintains client confidentiality. 

  • Identifies and meets with DCF personal involved with each of the families, helps to develop and meet with family support system regularly. 

  • Attends necessary meetings affiliated with the families. 

  • Completes necessary forms, records, and enters information into program databases. 

  • Keeps accurate records as required.

  • Keeps logs of activities and other necessary paperwork. 

  • Other duties as assigned based on program needs.

Skills & Qualifications:

A minimum of Bachelor’s Degree in Social Work, or related field with five years’ experience providing case management, family work, and working with children in a public or private human service or health agency or a degree in associated field with a minimum of 5 years’ experience providing case management and  family work.

  • Minimal 1 year experience working with child abuse cases and have a strong foundation of knowledge/experience working with families where there is mental health issues, domestic violence, and/or substance abuse.  

  • Must have a sound knowledge of services available to families in the Merrimack Valley. 

  • Ability to understand, apply, explain, and interpret program procedures for application and rules for service eligibility. 

  • Ability to maintain accurate records and confidentiality of information. 

  • Knowledge of and experience in motivational interviewing techniques. 

  • Ability to interact and communicate with clients under physical and/or emotional stress. 

  • Ability to establish rapport with persons from different ethnic, cultural, linguistic and/or economic backgrounds. 

  • Ability to function outside the Agency office in the neighborhoods of Greater Lawrence.

  • Some nights and weekends required. 

  • Must have a valid license and means of a car.

  • Computer skills a must.

  • Bilingual (English/Spanish) preferred.

  • Ability to work effectively with and meet the needs of people from a diverse range of backgrounds, cultures, and identities, including underserved communities and people from the different racial groups, socioeconomic statuses, nationalities, and languages that make up GLCAC’s clientele, to support the agency’s mission of building a more diverse, equitable and inclusive organization for employees and clients alike.
