Sevita Case Management Social Worker in Littleton, Colorado

NeuroRestorative, a part of the Sevita family, provides rehabilitation services for people of all ages with brain, spinal cord and medically complex injuries, illnesses and other challenges. In a variety of locations and community-based settings, we offer a range of programs, including vocational and therapy programs, day treatments, and specialized services for infants, children, adolescents, Military Service Members and Veterans.

Social Workers are responsible for the evaluation and treatment of clients admitted for treatment. The Social Worker adheres to specific scope of service for activities related to the evaluation and treatment of clients in the home and community.

ESSENTIAL JOB FUNCTIONS

To perform this job successfully, an individual must be able to satisfactorily perform each essential function listed below:

  • Evaluation including gathering data from client records, interviews, observation and dialogue with other team members

  • Development and implementation of a treatment plan according to physician orders which includes establishing client goals, continuous re-evaluation of progress and appropriate modification of the plan

  • Timely preparation of written documentation of client progress including initial evaluations, progress reports, discharge summaries and other requested reports

  • One to one counseling with client/and or family members in their home or other appropriate setting

  • Increases awareness of client and caregivers about brain injury to better facilitate adjustment and understanding of the process of recovery.

  • Attends team conferences and reports on client’s progress and current status in therapy as well as collaboratively assisting the team in identifying realistic goals

  • Completes and turns in billing information in a timely manner

  • Communicates effectively with internal case manager

  • Performs other related duties and activities as required.

Minimum Knowledge and Skills required by the Job

The requirements listed below are representative of the knowledge, skill, and/or abilities required to perform the job:

Education

  • Master’s Degree in Social Work from an accredited school of social work

  • Membership in NASW and related clinical experience in a rehabilitation setting with focus on traumatic brain injury and spinal cord injury.

Certificates, Licenses, and Registrations:

  • In MI, certified by the State of Michigan as a social worker

  • Valid Michigan Driver’s license for employees in state of Michigan

Other Skills and Abilities:

  • Motor Vehicle in operating condition

  • Ability to operate a motor vehicle

  • Able to travel distances exceeding 45 minutes

Other Requirements:

  • Travel as needed

Physical Requirements:

  • Medium Work. Exerting up to 50 pounds of force occasionally, and/or up to 30 pounds of force frequently, and/or up to 10 pounds of force constantly to move objects.
