    About the Position

    TheCase Manager,under the direction of the Project Manager, is expected to provide assistance and aids individuals referred through the California Department of Justice (DOJ) system.

    Core Duties (but not limited to) Case Manager :

  • Promote and live out the LifeSTEPS workplace values of integrity, innovation, empowerment, respect and personal growth in tandem with the OZ Principle.

  • Assess mental health service needs of clients, provide link to Full-Service Partnership (FSP) services

  • Provide crisis intervention and safety assessments to clients on caseload

  • Participate in court hearings and submit court progress reports

  • Weekly coordination with client’s multi-disciplinary team which might include psychiatrists, judge, probation officer, FSP team and DHS partners

  • Assist with medication management

  • Document all client contacts in PIP format (Purpose, Intervention, Plan) and in accordance with Housing Department productivity expectations

  • Must be physically at each assigned property as scheduled and spend the required contracted hours at each assigned property per month.

Qualifications

Education and/or Experience and/or License Certification:

  • Bachelor’s degree in social work, psychology or related field.

  • Minimum of 2 years’ case management or ICMS experience.

  • An equivalent combination of education and experience may substitute for the degree.

  • Certification as an AOD counselor desired

  • Familiar with Harm Reduction, Motivational Interviewing, and Housing First principles and practices.

  • Previous experience working with the homeless and those with severe mental illness.

  • Familiarity with the criminal justice system.

  • Valid California Driving License and reliable, insured transportation required
