Substance Use Disorder Counselor III in
Los Angeles, California
- Will assume responsibility for case management for clients assigned to caseload.
- Completion of ASAM assessment in conjunction with LPHA.
- Development of clinically appropriate treatment plan and updates in conjunction with client and clinical staff.
- Will provide support to Counselor I and II as appropriate.
- Provision of group, individual counseling and other supportive services which are appropriate to the client treatment process.
- Will facilitate education and awareness sessions on topics appropriate to recovery
- Responsible for writing client progress reports.
- Responsible for comprehensive chartingvia SAGE electronic medical records of all treatment and other services provided to the client, in accordance to applicable guidelines.
- Responsible for client discharge planning.
- Provision of case management services or referral for case management as necessary to contribute to patient's recovery through services linkages.
- Will be responsible for attending meetings and training sessions
- Other services as may be assigned and are appropriate to the position.
