As a Care Manager, you will be responsible for a wide range of duties aimed at improving the quality of life for our members, who may be older adults and/or individuals with intellectual and/or physical disabilities. Your key responsibilities will include assessment, member-centered care plan development, ongoing care coordination, quality assurance, and monitoring of services. You will conduct home visits, develop and maintain care plans, and educate members and their families on various issues related to aging, disability, or health. This role requires managing a caseload of program members in collaboration with an RN Care Manager, completing comprehensive member assessments, and providing ongoing care coordination. You will also ensure that documentation meets required standards and participate in continuous improvement efforts to enhance service quality.

Work Site

This is a hybrid position requiring some days on-site in Madison, Wisconsin and some days remote.

Work Environment

The position is based in Dane County with initial training in Milwaukee/Brookfield for 2-3 days. The role involves regular local travel for home visits primarily in Dane County, and occasionally in Jefferson and Rock County. The job allows for a hybrid work schedule after an initial training period, with a requirement to be in the office approximately 3 days per week. Travel requires the use of a personal vehicle, a valid driver's license, and proof of required insurance coverage. Work hours are generally Monday through Friday, from 8 a.m. to 4:30 p.m.
