U W-MADISON DIV. CONTINUING STUDIES Disease Prevention Care Coordinator in MADISON, Wisconsin

JOB REQUIREMENTS: Job Summary: The Disease Prevention and Care Coordinator will work within the Bureau of Communicable Diseases, Harm Reduction Unit, Harm Reduction Section to provide subject matter expertise, technical assistance, and outreach support on hepatitis C, HIV, and STIs. This position will work closely with the Harm Reduction Response Team to provide rapid and confirmatory hepatitis C and HIV testing to participants engaged in harm reduction services, educate participants about disease processes and transmission, and will work to present this information through a harm reduction lens. The Disease Prevention and Care Coordinator will assist HRRT participants in care navigation and be knowledgeable about medical resources throughout Wisconsin. The Disease Prevention and Care Coordinator will supplement ongoing communicable disease surveillance and intervention efforts within the Bureau of Communicable Diseases that focus on priority populations. These included justice-involved persons, pregnant persons, infants who have been perinatally exposed to communicable diseases, and persons living with co-diagnoses of HIV, hepatitis C, and/or syphilis. Justice-involved persons are a population of focus for Wisconsin\'s hepatitis elimination planning, and the Disease Prevention and Care Coordinator will be expected to continue outreach work with jails across the state to provide hepatitis C testing. This position will also have the unique opportunity to provide technical assistance to harm reduction agencies across Wisconsin in regard to service development and expansion, rapid HCV and HIV testing integration, and program development. This role will also provide technical assistance to multiple teams within the Harm Reduction Section on an ad hoc basis. This role will work closely with the Viral Hepatitis Program Coordinator, the Hepatitis C Epidemiologist, and the Hepatitis C Surveillance Specialist within BCD and HIV Surveillance Teams to remain up-to-date on the latest disease trends in Wisconsin.
