    The Agency for Persons with Disabilities works in partnership with local communities to support people with developmental disabilities in living, learning, and working in their communities. APD provides critical services and supports for customers with developmental disabilities to reach their full potential. The Agency serves people with spina bifida, autism, Down syndrome, children ages 3-5 at high risk of developmental disabilities, cerebral palsy, Prader-Willi syndrome, Phelan-McDermid syndrome, and intellectual disabilities.     SALARY RANGE

    $ 31,200.00 - $33,247.10 Annually $ 1,200.00- $ 1,278.75 Bi-weekly

    SPECIFIC DUTIES AND RESPONSIBILITIES

    Employees of the Agency for Persons with Disabilities are required to demonstrate total commitment to outstanding customer service. This includes, but is not limited to, always acting in a responsive, professional, courteous manner with the employees, customers, and public we serve at all times.

    Duties and responsibilities include, but are not limited to, the following:

    This is a professional position, which involves providing intake informational support to individuals with special needs, resulting from a developmental disability. The Human Services Program Analyst (intake) is responsible for maintaining intake caseload to determine APD eligibility. Consults with individuals, families and others regarding the APD eligibility requirement, services offered, waitlist and their categories, and the application process. This position requires travel to community locations.

    Complete the initial screening form, explain the intake process as requested by the community through phone, email, mail and or walk-in 100% of the time. Ensure information gathered for each intake is posted and/or emailed to area supervisor/Lead.

    Process all assigned APD application packets and ensure all are mailed out within twenty-four (24) hours of completing the screening form and document the application process within system tracker.

    Conduct quality review of documentation by obtaining copies of applicant's documents such as birth certificates, social security cards, Medicaid numbers and collateral Information to be submitted to APD Region Office to determine eligibility for APD services and possibly transfer to Wait List.

    Perform updates of all progress notes for each applicant's file and generate the required Notice of APD Eligibility/Ineligibility within (5) five calendar days in the APD electronic records system – iConnect.

    Initiate and complete the short version support plan once eligibility approval has been determined. The intake coordinator will have 45 days after the application has been signed by the individual and/or their legal guardian to complete the support plan and the required initial Home Community Based Eligibility Worksheet.

    Perform input, update and accurately maintain all referral, applicant information and forms in the iConnect and or other electronic systems utilized by the APD Agency within (5) five calendar days.

    Maintain and review all high-risk individuals' case files that are from 3-5 years of age on a monthly and quarterly basis to determine continuity of APD services by gathering more collateral information within sixty (60) days after the consumer has turned five (5) years of age.

    Perform other related duties or assignments.

    REQUIRED KNOWLEDGE, SKILLS, AND ABILITIES

  • Ability to plan, organize and coordinate work assignments.

  • Ability to travel for work purposes.

  • Ability to work under pressure, multi-task and independently.

  • Ability to establish and maintain effective working relationships with consumers, families, providers and staff within and across regions.

  • Ability to prioritize and schedule work efficiently, determine work priorities and ensure completion of work assignments.

  • Demonstrate skills in using Microsoft Office programs, applications and Web based programs.

  • Knowledge of experience working with Developmental Disabilities or Mental Illness, or the principles and practices of one of the allied health field principles or one of the rehabilitative sciences.

  • Experience interviewing applicants for public benefits assistance and services.

    POSITION REQUIREMENTS

    Valid Florida Driver License or other efficient means of transportation and willing and able to travel for work purposes.

    Preference for fluent bilingual - Creole, Spanish speaking and writing

    As a condition of employment, a person appointed to a position in the State Personnel System is required to participate in the Direct Deposit Program. Rather than receiving a paper paycheck, your funds will be deposited directly into your account at your financial institution. This will be accomplished by Electronic Funds Transfer. Banks, savings and loan associations, and credit unions are eligible to accept such deposits. Retirement funds are also required to be in the Direct Deposit Program.

    BACKGROUND SCREENING REQUIREMENT

    It is the policy of the Florida Agency for Persons with Disabilities that applicants for employment undergo Level 2 employment screening in accordance with the requirements of Chapter 435, Florida Statutes, as a condition of employment or being permitted to serve as a volunteer.

    No applicant for a designated position will be employed or permitted to volunteer until the Level 2 screening results are received, reviewed, and approved by the Agency.

    Level 2 background screening shall include, but not be limited to, fingerprinting for Statewide criminal and juvenile records checks through the Florida Department of Law Enforcement, and Federal criminal records checks through the Federal Bureau of Investigation and may include local criminal records checks through local law enforcement agencies.

    Pursuant to S. 110.1127(1), Florida Statutes, this position requires a background investigation, including fingerprinting.

E-verify is a program that electronically confirms an employee’s eligibility to work in the U.S. after completing the employment eligibility verification form (I-9).
