Amergis Care Manager - Dane County in Milwaukee, Wisconsin
The Non-Clinical Case Manager coordinates referrals, serviceplanning, and client documentation for designated caseload(s).
Essential Duties and Responsibilities:
Maintains full caseload, complete with ongoing individualservice plans
Assess ongoing changes in behavior, circumstances, orconditions that may affect client safety
Engages /involves client and sponsor in the casework process
Prepares, compiles, and submits accurate case files
Serves as a liaison with stakeholders (i.e.. legalproviders, immigration court)
Actively participates in seminars, workshops, educationalprograms
Completes home visits to meet with sponsor (if applicable)
Maintains contact with clients through phone calls and homevisits (if applicable)
Consults with supervisors to determine decisions as theyrelate to the safety, well-being and permanency of patient
Demonstrates client progress toward risk reduction,achievement of treatment goals and positive case outcomes
Maintains accurate records, files, forms, statistics andadditional relevant information in accord with agency policy, licensing, and/orfunding requirements
Conducts in-home assessments (if applicable)
Performs other duties as assigned/necessary
Minimum Requirements:
Professional License in Behavioral Sciences, Human Services,or Social Services preferred
Bachelor’s Degree in Behavioral Sciences, Human Services, orSocial Services fields preferred
Experience in Case Management, Behavioral Sciences, HumanServices or Social Services preferred
Complies with all relevant professional standards ofpractice
Participation and completion of Amergis' Competency programwhen applicable
Current CPR if applicable
TB questionnaire, PPD or chest x-ray if applicable
Current Health certificate (per contract or stateregulation)
Must meet all federal, state and local requirements
Successful completion of new hire training as applicable tojob site
Understand patient confidentiality and HIPAArequirements
Ability to effectively elicit/provide information to andfrom appropriate individuals (including, but not limited to, supervisors,co-workers, clients) via strong communication skills; proficiency in theEnglish language is required
Computer proficiency required
Must be at least 18 years of age
