Peer Specialist

The Link-Age Program aims to pre-empt and address the isolation of older adults by targeting people age 55+ whose independence, tenure, or survival in the community is in jeopardy because of their behavioral health challenges. The staff provides intensive case management, in-home visits, supportive counseling, and education/linkage to long term care and community support services using a holistic approach. The Peer Specialist will create additional support for individuals enrolled in the program that are having difficulty connecting to services.

Responsibilities

  • Provide a peer-to-peer relationship to support individuals in recovery and family members with sustaining recovery and overall wellness so they can achieve their goals with in-person visits at the office, at client's home, or in the community and/or with telehealth services.

  • Assist program staff with implementing the client's individualized care plan goals. Utilize program assessment tools to monitor the client's mental health/substance use and care needs.

  • Provide information and referrals to individuals and families for social and recovery support services, counseling, and linkages to prevention, treatment, and harm reduction services.

  • Participate in supervision and weekly Team meetings; participate in agency trainings and workshops, OMH trainings and other county trainings, which would be beneficial to position.

  • Prepare and maintain client case records and statistics as required by the standards set by Family and Children's Association/LinkAge grant guidelines.

  • Outreach within the community to increase awareness of program and services, provide information and assistance. Support agency efforts to reduce stigma and educate community members on mental health and substance use issues and services.

  • Facilitate groups at local senior centers to reduce isolation and increase education about mental health and substance abuse/misuse.

  • Assist clients with technology education to help connect them with supportive services.

Qualifications

  • High school diploma or equivalent required.

  • Bilingual Spanish a plus.

  • Completion of OMH NYS Certified Peer Specialists certification (NYCPS) or NYS Certified Recovery Peer Advocate (CRPA) certification

  • Valid NYS Driver's License required.

  • Lived experience required.

Location: Garden City

Schedule: Mon-Fri 9am-5pm, flexible

Salary: $23.50
