Roper St. Francis Psychosocial Patient Navigator Clinical Therapist in Mt. Pleasant, South Carolina

THIS POSITION LOCATED AT WEST ASHLEY CANCER CENTER. MAY ALSO FLOAT TO BERKELEY HOSPITAL, MOUNT PLEASANT HOSPITAL, AND TRICOM NORTH CHARLESTON.

HOURS: 8:00am - 5:00pm

Primary Function/General Purpose of Position

The Clinical Therapist’s role in the continuity of care of patients includes an understanding and participation in the treatment of patients with psychiatric problems following established standards as outlined by departmental policy and procedure manuals and professional licensing agencies.

Essential Job Functions

  • Assist the patient in successfully attaining and maintaining therapeutic goals by: · Providing direct inpatient and outpatient care · Assess the patient’s psychosocial history as appropriate · Design and tailor therapeutic interventions specific to each patient. · Collaborates with the interdisciplinary team in developing patient treatment plans. · Attend interdisciplinary team staffing sessions for the purpose of reviewing patient goals, make modifications, if and when necessary. · Collaborates with the interdisciplinary team, the patient, and the family (when possible) in developing effective patient discharge plans and placement. · Liaison with family members on patient’s progress and educating family members on changing their behaviors to assist patient going forward after discharge, as well as discharge planning

  • Liaison with the Probate Court for involuntary admissions; provide progress reports and recommendations as needed.

  • Documents progress, interventions, and resources engaged in discharge planning

  • Other secondary duties include public relations activities and community/hospital staff education.

Licensing/Certification

  • Currently licensed as a Professional Counselor or Social Worker

  • If candidate/employee is licensed as a Professional Counselor-Intern, documentation of the supervising Licensed Counselor must be on file (does not have to be a RSFH employee).

  • Must have a current American Heart Association BLS for Healthcare Provider Card (not required for position located in oncology dept.).

  • Non-Medicare billing departments include: 50-8084 RSF Primary Care-Rivers Ave, 12-3024 NSG Behavioral Medicine, and 10-3598 340B HIV Program.

Education

  • Master’s degree in Mental Health field or Graduate degree in a directly related counseling program (ex. Ed.S in Counselor Education) from an accredited college (required)

Work Experience

  • Two years of psychiatric or counseling experience (required)

Working Conditions/Physical Requirements

Frequent sitting, walking standing. Intermittent stooping or bending. Potential for injury with combative or unpredictable patients. In addition to lifting of patients, may require lifting, carrying or moving items up to 50 lbs. Frequent use of finger/hand dexterity. Constant talking or hearing. Corrected hearing and vision to normal range. Minimal exposure to blood, body fluids or tissue. Possible exposure to infectious materials, communicable diseases and/or other conditions common to a healthcare environment. Requires work under stressful conditions, deadlines, and/or irregular hours. Ability to read and comprehend.

Hard/Tech/Clinical Skills :

  • Demonstrates a high level of knowledge of the mental health categories of illness and behaviors exhibited by patients.

  • Demonstrates a high level of knowledge and skills in the therapeutic treatment of patients to include the facilitating of psychotherapy and psycho-educational groups.

  • Provides a high level of skill in crisis intervention management of patients.

Soft/Interpersonal Skills:

  • Demonstrates professional communication with medical personnel, family members and community agency personnel.

  • Demonstrates an ability to be flexible when interacting with coworkers, patients, patient’s family members, and to work as an interdisciplinary team member.
