Job Description:

We are looking for an experienced Full-Time Nurse Practitioner to join our Family Medicine team. The ideal candidate will provide primary healthcare services to patients of all ages, offering preventive care, managing chronic illnesses, and providing nutritional counseling. Our patient-centered approach allows us to personalize treatment for our patients and their families.

Key Responsibilities:

  • Provide high-quality, patient-centered care in a family medicine setting.

  • Conduct comprehensive health assessments, including patient history and physical examinations.

  • Diagnose and treat acute and chronic illnesses.

  • Prescribe medications and other treatments as necessary.

  • Order and interpret diagnostic tests and laboratory results.

  • Develop and implement individualized treatment plans for patients.

  • Educate patients and their families on disease prevention, health maintenance, and management of chronic conditions.

  • Collaborate with physicians, nurses, and other healthcare professionals to ensure coordinated and comprehensive care.

  • Maintain accurate and up-to-date patient records and documentation.

  • Participate in quality improvement initiatives and contribute to the development of clinical guidelines and protocols.

    Qualifications:

  • Current licensure as a Nurse Practitioner in New Hampshire.

  • Certification as a Family Nurse Practitioner (FNP) by a recognized national certifying body.

  • Minimum of 2 years of experience in a family medicine or primary care setting.

  • Excellent clinical, diagnostic, and decision-making skills.

  • Strong communication and interpersonal skills.

  • Ability to work independently and as part of a multidisciplinary team.
