The Position

Health Care Coordinators perform professional work related to Medicaid program operations and auditing providers to ensure compliance with program policies and regulations.

Incumbents perform in a trainee capacity and acquire the knowledge, skills and abilities required in this occupation. The Aging and Disability Services Division is currently recruiting for a full-time Health Care Coordinator 1 in Reno/Sparks, Nevada. This recruitment will be used to underfill positions in the Health Care Coordinator series. Advancement to the next level may occur after meeting the minimum qualifications, satisfactory performance, and with endorsement of the appointing authority. This recruitment may be used to fill current and future vacancies as they occur in the following Program: Home & Community Based Services (HCBS).

Incumbents oversee caseloads of program participants based on slot allocation, staffing ratios and manage a caseload of participants residing in urban/rural areas. To offer participants with case management, incumbents will evaluate each person and set up continuing services in accordance with Person Centered Plans of Cares, monitor program services, and help or refer clients to the proper community resources. Incumbents will contact clients via phone, email, and/or in-person interaction. Find and organize a network of resources and services to address the health and safety requirements of the participant. Promptly enter program data into the appropriate databases and submit the necessary reports to the appropriate state, county, or local government agencies. To Qualify:

In order to be qualified, you must meet the following requirements:

Education and Experience (Minimum Qualifications)

Licensure as a Social Worker or professional licensure or certification in a medical specialty applicable to the assignment.

Special Requirements

A valid driver's license is required at the time of appointment and as a condition of continuing employment. A State of Nevada/FBI background check will be required of the selected applicant. Professional licensure or certification as a social worker, nurse or in a medical specialty issued by the appropriate licensing board in the State of Nevada at the time of appointment. Incumbents must maintain professional licensure or certification in their specialty as a condition of continuing employment. Travel up to 50% of the time is required.
