UPMC OP Nurse Coordinator I in New Castle, Pennsylvania

UPMC is hiring a full-time OP Nurse Coordinator I to join their Physician Services team located at Westgate Plaza, New Castle. In this position, you will work Monday-Friday 8:00am-4:30pm- No weekends or holidays required!

*May be required to travel to other UPMC locations to assist in staffing needs

Key Responsibilities:

The Professional Nurse Coordinator will demonstrate accountability for professional development that enhances the quality of practice and patient care. They will actively participate in practice-based shared governance, goal setting, and support the change and transition process to improve care quality and the practice environment. As a highly engaged and full partner on the care team, the Nurse Coordinator will respond willingly to team member needs for assistance and partnership, participating in work that enhances patient care and the professional practice environment.

The role involves adapting to change and demonstrating flexibility throughout the change process. The Nurse Coordinator will provide oversight and supervision to the overall practice environment, assisting all other disciplines and directing clinical and administrative activities as needed. They will exhibit leadership capabilities with new nursing staff, patient information coordinators, medical assistants, and office assistant staff in the daily management of the patient process.

Patient Care:

The Nurse Coordinator will formulate goals and a plan of care for patients, involving them as partners and considering their individual needs holistically. They will triage patient situations and coordinate care with physicians, assisting with all aspects of patient care, including assessment, evaluation, and education. The Nurse Coordinator will provide care for patients in a global, holistic manner, maintaining responsibility for the patient beyond office or procedural visits. They will be accountable for outpatient treatment follow-up necessary for patients to maintain health between visits and serve as the primary nursing resource for their caseload. The role also involves facilitating necessary services outside the healthcare facility and interacting with homecare and other support services.

Professional Relationships and Development:

The Nurse Coordinator will develop and maintain productive working relationships internally and externally, demonstrating accountability for actions, enthusiasm, motivation, and commitment to patients and colleagues. They will understand cultural differences and hold peers accountable for healthy relationships, maintaining a balance between work and personal life. The Nurse Coordinator will model safe work hours, time management, and a healthy lifestyle, communicating any safety hazards identified in the workplace to peers and management. They will assist the departmental manager in patient care planning, operations, and innovations.

Quality Improvement and Advocacy:

The Nurse Coordinator will actively participate in department or practice-specific quality improvement efforts, identifying opportunities for improvement to colleagues and management. They will take personal responsibility for improving patient satisfaction with the quality of care and service, utilizing research and evidence-based practice to support clinical care improvements. The Nurse Coordinator will assist in preparation and maintenance activities for regulatory agency reviews and quality audits, becoming the patient advocate and voice to physicians and physician extenders within the practice.

Skills and Qualifications:

The ideal candidate will have the ability to establish and maintain positive, caring relationships with executives, managers, physicians, non-physician providers, ancillary and support staff, other departments, and patients/families. They will work productively and effectively within a complex environment, handling multiple and changing priorities and specialized equipment. The role requires good clinical judgment, critical thinking, analytical, and problem-solving abilities related to various aspects of patient care. The Nurse Coordinator will demonstrate critical thinking in identifying clinical, social, safety, psychological, and spiritual issues for patient care within an episode of care. They will practice solid communication skills, articulating and translating the patient's condition to other care providers and making recommendations for changes in patient care and office practices. Patient documentation will be comprehensive, promoting communication between caregivers.

Education and Training:

The Nurse Coordinator will demonstrate knowledge of adult learning principles (and/or teaching children if applicable) and apply them in teaching patients, families, students, and new staff. They will provide detailed and appropriate teaching to patients and families to guide them through the episode of care both in the office setting and via phone or written communication when at home. The Nurse Coordinator will serve as the primary nursing liaison for developing a total care plan for patients and families, supporting the development of students, new staff, and colleagues. They may serve as a preceptor for newly acquired staff and be responsible for coordinating staff training and educational activities. The role involves functioning as the senior nursing level, assisting in the professional development and maturity of new staff members and ancillary support. The Nurse Coordinator will create an environment of open dialogue, inquiry, and continuous development by asking for feedback and improving practice.

  • Minimum 3 years nursing experience

  • BSN preferred

Licensure, Certifications, and Clearances:

  • Current licensure as a Registered Professional Nurse in practicing state

  • CPR required based on AHA standards that include both a didactic and skills demonstration component within 30 days of hire

  • Basic Life Support (BLS) OR Cardiopulmonary Resuscitation (CPR)

  • Registered Nurse (RN)

  • Act 31 Child Abuse Reporting with renewal

  • Act 33 with renewal

  • Act 34 with renewal

  • Act 73 FBI Clearance with renewal

*Current licensure either in the state where the facility is located or, if the facility is in a state covered by the multistate Nursing Licensure Compact (NLC) agreement, a multistate license issued by a participating NLC state. Hires and current employees working on an out-of-state NLC license who later change their residency to the state where the facility is also located will have 60 days upon changing their residency to apply for licensure within that state.
