Full-time LCSW Behavioral & Mental Healthcare

Nuvance Health, formerly known as Western Connecticut Health, has as excellent opportunity for a full-time, experienced Licensed Clinical Social Worker (LCSW) working in New Milford, Connecticut. The LCSW will provide support to the Behavioral and Mental Healthcare department following a Monday-Friday office schedule of 9:00am-5:30pm. We offer a highly competitive compensation package including base salary. This position provides clinical social work services and assessment, treatment, psychotherapy to address and meet patients' emotional and social stress.

The ideal candidate will be a seasoned clinical social worker with strong broad based clinical skills, group work, crisis intervention, and discharge planning as well as hold a Connecticut state licensure as a Clinical Social Worker (LCSW). The position will be providing clinical services to emotionally ill individuals and in a group dynamic as well as collaborating with psychiatry and health professionals for the purposes of quality DSMIV diagnostic and treatment of mental illness, mood, psychotic, and personality disorders.

Job Responsibilities Include:

·Facilitates group therapy, provides individual, family, crisis intervention, patient-centered support and psycho-education

·Establishes objectives and develops patient-focused treatment plans to provide disposition and/or referral. Identifies potential barriers, limitations & lack of coping abilities, adjustments, and functional status. Participates in patient care planning

·Conducts intakes with patients/family members to evaluate coping abilities, dysfunctional adjustments, maladaptive behaviors, functional status

·Maintains knowledge of community resources and refers patients & families to agencies for supportive services & concrete resources (financial assistance; housing; transportation; medications) on regular & emergency basis

·Completes hospital and state required documentation & other paperwork & forms, including insurance authorization requirements

·Evaluates for child, elder abuse, and domestic violence and makes appropriate referrals. Acts as a consultant and/or educator to staff relative to assessment, criteria & referral

·Acts as a liaison for the patients within community (local, state and private agencies)

·Fulfills all compliance responsibilities related to the position

·Performs other duties as assigned

Qualifications:

·Master's degree in Social Work from an accredited school

·Minimum of 3 years’ experience

·Current CT Licensed Clinical Social Worker (LCSW)

·Prefer: Licensed Alcohol and Drug Counselor (CT)

·Experience in assigned specialty area, group work, crisis intervention, discharge planning, DSMIV

PREFER: Licensed Alcohol and Drug Counselor (LADC).

PREFER: Three years experience, both inpatient and outpatient. Group counseling skills.
