Description:
Home Health WOCN RN
Position Purpose: The Home Health WOCN RN provides expert nursing care in wound, ostomy, and continence management to home care patients. They collaborate with healthcare professionals and patients/families to ensure optimal outcomes.
Qualifications:
Graduate of an accredited nursing program (BSN preferred)
Current RN license in the state of practice
Certified Wound Ostomy Continence Nurse (CWOCN) certification required
Minimum 2 years of RN experience in a clinical setting
Proficiency in electronic medical records systems
Strong assessment, critical thinking, and communication skills
Ability to work independently and as part of a team
Valid driver's license and reliable transportation
Current CPR/BLS certification
Essential Functions:
Conduct comprehensive assessments of wounds, ostomies, and continence issues
Develop individualized care plans with the interdisciplinary team
Educate patients and families on wound care, ostomy care, and continence management
Provide direct patient care and monitor progress
Participate in quality improvement activities and maintain documentation
Provide in-service education and participate in continuing education
