Granville Health System Medication Reconciliation Technician in Oxford, North Carolina

Position Summary Responsible for accurately collecting and entering patient medication histories into the hospital#s information system#to be used by physicians and other health care providers to determine medication orders during the hospital admission and upon discharge from the hospital.#This could include contacting retail pharmacies and/or physician offices to verify the medication history to achieve the highest level of accuracy.#Will also collaborate with admission/discharge nurses and pharmacists as needed to resolve medication history related issues. Observes#GMC#s attendance policy.# Complies with departmental policies for reporting to work on time and for complying with designated meal/break period time frames.# Attends and participates in department meetings. Identifies self in a pleasant and positive manner.# Assists patients, family members or other clients with concern and empathy.# Demonstrates effective communication, recognizing diversity among age groups, cultures, and educational levels. Works consistently in a positive and cooperative manner with other employees in and outside the departmental unit.#The medication reconciliation technician may provide other pharmacy services within the scope of their license, certification, registration, and/or assessed competencies. Practice will be in accordance with laws and regulations.# Qualifications High school diploma or GED. Certified or Registered Pharmacy Technician.# Minimum one year retail pharmacy experience including verification and clarification of prescription medication orders and/or hospital medication reconciliation processes and experience with order entry software. #Meditech experience preferred. Must be able to work under the direction of a Pharmacist and follow written or verbal instructions effectively.# Participates#in educational conferences, completes#education in-services, #and maintains knowledge and skills related to areas of practice.
