Description
Principal Accountabilities:
Perform individual and group counseling services under the guidance of, and within the scope of practice, the supervising provider
Conduct services with regard to the Quality Improvement program, assuring compliance with all local, state and federal requirements
Adhere to all staff by-laws, administrative and personnel policies and procedures
Refer severe cases to appropriate community or medical agencies
Assist in developing and maintain principles of practice and clinical policies for ConnextCare
Refer appropriate cases to other specialists for evaluations as needed (i.e. speech/language/reading, hearing, and vision)
Assist in developing behavioral portion of annual health care plan, which addresses data collection within the electronic health record
Work with the Chief Medical Officer in determining patient satisfaction and acting on those issues that may improve the delivery of health care
Establish contact with parent to inform counseling taking place and establish support, when applicable
Develop parental resources or provide family counseling, as needed
Maintain proper and accurate patient record, complying with all local, state, and federal requirements.
Complete encounter forms and code appropriately indicating services rendered
Participate in chart audits and peer review for purposes of quality assurance
Meet with behavioral supervisor on a scheduled basis for case review/management
Participate in applicable organization Scheduled Meetings
Work with the management to carry out Federal guidelines and to communicate with representatives from the Health Resources and Service Administration, participating in the grant process if requested by the President/Chief Executive Officer.
Engage in Patient Centered Medical Home initiatives, including participating as part of the Clinical Care Team
Other duties and responsibilities as requested by management and supervisor.
Requirements
Knowledge/ Education Requirements:
A. Graduate of an accredited Social Work school program
B. Valid/active Master Social Worker license to practice in New York State
C. Promoting the organization’s code of conduct, safety rules and adherence to all company policies
D. Carry out job responsibilities in an ethical, effective and professional manner
E. Cooperate, work, and communicate with staff, supervisors, management, and patients in a professional manner
F. Computer proficiency with Electronic Health Record
G. Ability to display compassion and work well in stressful situations
H. Must be keenly aware of the importance of confidentiality in all aspects of the position
Salary Description
$56,000-67,760/year depending on experience
