University of Rochester Allergy, Immunology and Rheumatology (AIR) - Improving Quality in LUPUS (IQ-LUPUS) - RN, Nurse Navigator in Rochester, New York

Responsibilities

Seeking a team-oriented nurse for an exciting new opportunity at AIR in Rheumatology as part of our IQ-LUPUS program which focuses on improving the quality of life for the LUPUS patient. The IQ-LUPUS Nurse Navigator removes barriers and connects LUPUS patients to resources and information to make informed decisions thorough assessment of the emotional, clinical, financial, psychological, and spiritual needs of the patient. The IQ-LUPUS Nurse Navigator also serves as a patient advocate, acts as a liaison between the patient and clinical care staff and helps to guide and provide care coordination to patients and their families as they navigate our complex healthcare system. Responsibilities include some direct patient care, telephone management, patient triage, connection with outside services such as social work, guidance and education for new diagnoses, medications and treatment plans and coordination of care with a focus on LUPUS and some other Rheumatology patients within the team of providers. Ideal candidates will have experience with phone triage and management of in-basket patient messaging. Must possess strong interpersonal, assessment and communication skills, be creative, proactive, and independent in decision-making and have good organizational skills.

Qualifications

Bachelor in Nursing required; if currently an Associate's Degree RN, nurse has 5 years to complete the BSN. Three to Five years of Nursing experience required or equivalent combination of education and experience. Required competence in critical thinking, interpersonal relationships and technical skills. Licensure as a Registered Nurse in NYS; certification in a Nursing specialty area preferred.

Pay Range

Pay Range: $36.37 - $47.28

The referenced pay range represents the minimum and maximum annual salary for this faculty position. Individual salaries will be determined within the job’s salary range, and will be determined by considering factors including, but not limited to, market data, education, experience, qualifications, expertise of the individual, and internal equity considerations. Incentive-based compensation may also be applicable and will be discussed during the hiring process.
