Coordinate transition to success program. Conduct assessments, classes and prepare written reports for schools and/or other referral services as needed. Provide program services per contract ensuring milestones are achieved. Maintain student records in a timely manner as required by applicable standards. Serve as liaison and advocate with families and other agencies. Arrange transportation for students to job shadowing opportunities. Assure completion of follow-up of assigned students. Complete necessary records for outcomes management, billings, etc. Understand supportive and customized employment. Monitor student participation rate and conduct continuous recruiting efforts; attend IEP meetings if invited and serve on assigned committees as needed.
