Providence St. Joseph Health Clinical Social Worker MSW- Advanced Cardiac Support (ACS) in Seattle, Washington

Description The Advanced Cardiac Support (ACS) Clinical Social Worker serves as a core member of a multi-disciplinary collaborative team led by the ACS Medical Director and Program Director that may involve clinic nursing staff, system-wide care and case management staff, pharmacists, substance abuse specialists, and psychiatrists. The role of the social worker is to serve the needs of all ACS patients, including patients with durable left ventricular assist devices, which will require flexibility and an ability to provide different types and intensity of services. FTE: 0.8 case management / programmatic projects / nonbillable work 0.2 billable psychotherapy / patient counseling ESSENTIAL DUTIES & RESPONSIBILITIES : Facilitates appropriate patient referrals to, and coordination with, Home Health, Skilled Nursing Facilities, Palliative Care, and Hospice. Proactively addresses social work issues in an effort to reduce inpatient admissions, reduced lengths of inpatient stay, and limit patient morbidity and mortality. Provides psychosocial evaluation, treatment planning, individual therapy, group support, case management, discharge planning, and crisis intervention to Advanced Cardiac Support patients including patients with durable Left Ventricular Assist Devices (LVADs). Maintains written progress notes in patients' electronic medical record as documentation of services. Maintains a thorough knowledge of appropriate community resources and makes appropriate referrals to these agencies or services as needed. Utilizes appropriate cognitive/behavioral interventions in areas of anxiety reduction, mood management, and relaxation training. Utilizes other professionally accepted psychosocial interventions as appropriate to patient needs. Assures compliance with regulatory agencies including but not limited DNV-GL and CMS. Works collaboratively with the Advanced Cardiac Support Medical Director and Program Director to develop, implement and assessing clinical and administrative services and programs. Serves as a department resource for problem solving issues related to social work. Collaborate with other department to resolve issues, share information, and enhance effectiveness of services and patient care. Provides complete psychosocial assessment and care coordination for all left ventricular assist device (LVAD) patients in collaboration with the LVAD Coordinators and ACS Providers. Serves as a liaison for the ACS service with the inpatient social worker and case management. Required Qualifications: Master's Degree Social Work from a CSWE accredited School of Social Work. upon hire: Washington Advanced Social Worker License. Or upon hire: Washington Advanced Social Worker Associate License. Or upon hire: Washington Clinical Independent Social Worker License. Or upon hire: Washington Clinical Independent Social Worker Associate License. upon hire: Eligible for credentialing by Medicare, Medicaid and other Third Party Insurance Payers. 3 years Work related experience, preferably in a primary care clinic or hospital environment.
