DEPARTMENT DESCRIPTION: The After Care Clinic provides transitional care for recently discharged HMC Emergency Department and in-patients who require urgent follow-up but lack a Primary Care Provider. The After Care Clinic also facilitates referrals to Primary Care through the HMC Clinic system or community-based clinics.

PRIMARY JOB RESPONSIBILITIES: • Utilize the nursing process (assessment, planning, implementation and evaluation) to prescribe, prioritize, delegate, coordinate and provide safe, therapeutic nursing care to patients/families. • Make independent judgments/decisions and takes action regarding a wide range of routine and complex patient problems and situations. • Collaborate with other disciplines as appropriate to coordinate continuity of patient care. • As assigned by the AACS clinic practice manager and/or RN3, could assume the responsibility of charge nurse. • Participate in clinic and selected divisional activities to promote delivery, problem solving and communication and continued professional development for self and others.

REQUIREMENTS: • Active, valid Washington State or Multi-state RN license required by start date. • Two years of professional nursing experience.
