Harborview Medical Center has outstanding opportunity for a Temporary (up to 12 months, fixed duration) position for an RN to support the Community House Calls/Health, Education & Outreach program. The purpose of the Community House Calls/Health, Education & Outreach Nurse is to learn about the role of the Caseworker/Cultural Mediators and make recommendations about the creation of such a role at the UW level to support clinical trials.

This nurse is a highly skilled Nurse with community and acute care experience with strong written and verbal communication skills. This unique role encompasses building a trustful relationship within diverse communities utilizing the foundations of hospital-based experience. In this role, the RN will work closely with twelve Caseworker Cultural Mediators to develop best practices for program improvement and disseminate relevant community health education.

WORK SCHEDULE • Full-time, 100% FTE, 40hrs per week 60% support the community House Calls program, 40% liaise with the Office of Health Care Equity, the Institute of Translational Health Sciences (ITHS), and the Human Subjects Division (HSD) of the University of Washington and Fred Hutch Note: Work hours may exceed 40 hours per week and can be deadline dependent. • Monday - Friday • Day Shift • Anticipated to be up to 12 months in duration.

LOCATION: • Harborview Medical Center (Seattle, First Hill)

JOB RESPONSIBILITIES (to include but not limited to): • Function as a liaison between administration, physicians, nurses, and community members • Deliver direct nursing care as required during home visits, such as blood glucose and blood pressure monitoring, wound care, and medical equipment management • Accompany CCMs during home visits • Provide program education via presentations & case studies to HMC staff • Review case studies and identify HMC process improvement projects to mitigate system barriers • Collaborates with interdisciplinary team to ensure effective, culturally appropriate, and equitable health care delivery • Partner with EthnoMed during major public health events, including health fairs, to educate, inform, and support communities • Provide, under the supervision of the Manager of Program Operations, clinical support, and guidance to Caseworker Cultural Mediators • Evaluate and provide best practice recommendations for current Diabetes CCM interventions and workflow • Promote and provide educational information via presentations and hand-outs in literary-appropriate languages • Assess health care barriers with the collaboration of the CCM team & Manager • Develop interventions to bridge gaps in care among LEP patients • Provide health education information via handouts and presentations on relevant healthcare topics (Diabetes, HTN, Stroke, Social Services, etc.) • Serve as a liaison to community & public health nurses, social workers, and other health care professionals • Gather data for hospital improvement projects addressing healthcare equity • Assist with research facilitation if deemed necessary for the target community

MINIMUM QUALIFICATIONS Current WA RN license Minimum of three years of clinical nursing experience
