$5,000 Sign-On Bonus to eligible new employees joining UW Medicine HARBORVIEW MEDICAL CENTER SOCIAL WORK has an outstanding opportunity available for a SOCIAL WORKER - INPATIENT CARE MANAGEMENT BED READINESS PROGRAM .
WORK SCHEDULE • 40 hours per week • Day Shift
POSITION HIGHLIGHTS • This position will provide care coordination for an effective early discharge process. • This is a two-year funded State Project, focusing on Medicaid Fee-for-Service, Managed Medicaid, and Dual Medicaid-Medicare eligible adult clients who are medically ready to be transferred outside of an acute care setting but are unable to due to transition complexities and barriers. • The Social Worker, in conjunction with the patient and interdisciplinary care team, identifies the clinical and financial targets and milestones for discharge and helps to eliminate barriers as possible.
PRIMARY JOB RESPONSIBILITIES • Provide specialty consultation to staff and health care teams regarding psychosocially focused aspects of client/patient care • Provide specialty consultation to Care Management and other health care staff who provide referrals to the Enhanced Care Management (ECM) program • Provide psychosocial support to patients and families to facilitate discharge from an inpatient setting to an ECM followed post-acute setting • Follow patients in a post-acute setting and coordinate with the interdisciplinary team for ongoing clinical needs and discharge planning into the community or sustain community residence • This position is based out of HMC yet visits with patients may occur in a variety of settings: inpatient, skilled nursing facilities, community dwellings, shelters, etc.
REQUIRED POSITION QUALIFICATIONS A Master of Social Work Degree from a program accredited by the Council on Social Work Education AND One year of full-time social work experience in a health care setting or equivalent. Experience can include a practicum placement as part of a social work training program and/or employment experience.
LEGAL REQUIREMENT Within 90 days of hire must hold one of the following credentials issued by the state of Washington: • Agency Affiliated Counselor • Licensed Social Work Associate -Advanced • Licensed Social Work Associate - Independent Clinical • Licensed Advance Social Worker • Licensed Independent Clinical Social Worker
