Health Advocates Network Hospice LPN in Sioux Falls, South Dakota

Job Summary:

This position provides care to assigned clients in a caring, safe and efficient manner and is responsible to perform the following according to location standards, procedures and individualized client plans of care.

Essential Duties and Responsibilities:

Defines the unit of care (family and client). Demonstrates knowledge of hospice palliative care; hospice philosophy and scope of services; importance of coordinating services and continuity of care. Recognizes different cultural attitudes and values towards death and dying. Identifies the five stages of the grieving process; anticipatory grief; normal vs. abnormal grief and impact of grief on client's family. Provides care according to plan of care and evaluates clients' need for additional services. Involves client and family in creation of plan of care. Coordinates with interdisciplinary team as needed. Completes required documentation in appropriate timeframes. Explains advance directives; clinical requirements and interdisciplinary services available through agency. Works independently with minimal supervision and seeks guidance as needed. Performs initial client assessment and ongoing assessments of the client's status. Participates in creation of the plan of care. Ensures the plan of care is implemented by the interdisciplinary group. Processes physician services/orders. Completes quality assurance audits as assigned. Participates in agency in-services; educational opportunities; team meetings and committees. Assists with customer

experience/marketing activities to expand community reach.

Basic Responsibilities:

This position will be held accountable for complying with all related laws, regulations, company policies and procedures pertaining to his or her position and for fulfilling his or her obligations under the Facilities Corporate Compliance Program.

Qualifications:

Education, Experience, Licensure, Certification:

Graduate of an accredited school of nursing. Licensure for state of practice is required, CPR through AHA.

Skills/Abilities: Must be at least 18 yrs of age.

Knowledge:

Prior work experience in like or related position may be helpful.
