Homecare/Palliative/Hospice: * Provides supportive services to help meet the patient/family social, emotional, economic and environment needs in relation to the impact of a chronic, acute or terminal illness though education, support/counseling and referrals/coordination of community resources. * Provide patients/families with the psychosocial support needed to cope with chronic, acute or terminal illness through education, support/counseling and referrals for community resources. * In addition to conducting community visits, some work may be required in palliative care and working directly with the hospitalist team performing program development and inpatient consults at Lakeview Hospital. Work Schedule: 0.5 FTE - Wednesday afternoons (half day), Thursday (full day), Friday (full day). Program hours are from 08:00 am - 04:30 pm and weekends by department rotation Required Qualifications: Master's degree in social work Must have Minnesota (MN) or Wisconsin (WI) Social Work license upon start date MN must have one of the following: Licensed Graduate Social Worker (LGSW) or Licensed Independent Social Worker (LISW) or Licensed Independent Clinical Social Worker (LICSW) WI must have one of the following: Advanced Practice Social Worker or Licensed Clinical Social Worker If licensed in only one state upon hire, must be willing to obtain license in the other state and provide proof of license within 4 months of start. Must have vehicle, driver's license and current auto insurance.
