TIOGA MEDICAL CENTER (TMC) Patient Education (LPN/RN position) in TIOGA, North Dakota

QUALIFICATIONS Minimum requirements include all of the following:

Active LPN or RN license in good standing with the State of North Dakota.

1 to 2 years of experience as a Licensed Practical Nurse or Registered Nurse preferred.

ESSENTIAL FUNCTIONS

Act as a patient advocate by being a resource for clinic and hospital staff regarding services available to patients. Attend educational meetings such as drug representative luncheons and various community program meetings set up by the Clinic Manger. Stay up to date on changes to specialty providers in the region and services that are available.

Responsible for coordination of services; coordinate daily, weekly, and monthly services with Radiology for x-rays, CT, MRI, Ultrasound, and Dexa scan; track sleep studies; coordinate scheduling, preparation instruction, and reminder letters for outpatient treatments such as injections.

Responsible for the pre-authorization of services; pre-certify with insurance companies all radiology services ordered in the clinic and hospital setting; pre-certify with insurance companies, all medications that require pre-certification; complete concurrent review when necessary.

Responsible for coordination of referrals to other medical facilities for tests or to other physicians; must ensure that a copy of all tests performed, EKG's, lab work, x-ray films, pathology reports, pathology slides, and a letter of referral, is at the physicians office at the time of or before the referral.

Complete monthly chart review forms and submit to Quality Assurance Director. Work with QA Director and Clinic Manager to complete program evaluation and update chart review forms as needed. Track TMC and visiting provider visits and submit with chart review to QA Director. Send surveys monthly to patients and report responses to QA Director and Clinic Manager. Send visit count to Clinic Business Manager.

Assist and help on the nursing floor as needed.

May be required to travel to attend work-related meetings and educational opportunities.

Participates in continuing education.

Attends Department Head meetings.

Performs all duties or tasks requested by the CEO or supervisor.

Establishes a culture of compliance by adhering to all facility policies and procedures. Complies with standards of business conduct, and state/federal regulations and guidelines.

Follows appropriate safety and hygiene measures at all times to protect residents, patients, and themselves.

Maintains confidentiality of protected health information, including verbal, written, and electronic communications.

Reports noncompliance with policies, procedures, regulations, or breaches in confidentiality to appropriate personnel. Reports any retaliation or discrimination to HR or CEO.

Reports any allegations of abuse, neglect, misappropriation of property, exploitation, or mistreatment of residents to supervisor and/or administrator. Protects residents and patients from abuse and cooperates with all investigations.

Reports any occupational exposures to blood, body fluids, infectious materials, and/or hazardous chemicals in accordance with facility policy.

Participates in all life safety and emergency drills and trainings. Fulfills responsibilities as assigned during implementation or activation of the facilitys emergency plan.

Reports work-related injuries and illnesses immediately to supervisor.

Follows established infection control policies and procedures.

As a condition of employment, completes all assigned training and skills competency.

Must have the knowledge, skills, and abilities as required by Tioga Medical Centers Licensed Practical Nurse or Registered Nurse job description.

Must not pose a threat to the health and safety of other individuals in the workplace.

Must be able to move intermittently throughout the workday.

Ability to sit for prolonged periods of time at a desk while working on a computer.

Ability to maintain good balance, stand for prolonged periods of time, and have good hand dexterity.

Ability to bend, stoop, kneel, crouch, perform overhead lifting and perform other common physical movements as needed for the position.

Meets general health requirements according to facility policy, including medical and physical exams and checking immunity status to various infectious diseases.

Ability to assist in evacuation of residents and patients during emergency situations.

Subject to exposure to infectious waste, diseases and/or conditions which include AIDS, Coronavirus, Hepatitis B, and Tuberculosis.

May be subject to hostile or emotional residents, patients, family members, visitors or personnel.

Ability to work independently, and to organize, plan, and manage time effectively to complete assignments.

Knowledgeable of nursing and medical practices, and ability to demonstrate knowledge and skills necessary to provide appropriate care to meet resident and patient needs.

Effective verbal and written communication skills and ability to exercise judgement.

Intermediate computer skills, including ability to create documents in various programs (Word, Excel, Publisher, PowerPoint, Adobe), and navigate electronic medical record systems.

Knowledgeable of programs available to residents of surrounding communities and how to make referrals.

Must be able to follow oral and written instructions.

Works in well-lit, ventilated area.

Subject to frequent interruptions.

Communicates with medical and nursing staff, and other departments.

Subject to call-back during emergency conditions.
