Description

The Bilingual Spanish Case Manager is responsible for delivering exceptional customer service and support to clients and their families, facilitating access to treatment and additional resources through professional assessment, service planning, and advocacy. This role is essential in addressing attendance barriers, supporting agency engagement activities, and providing assistance to other departments as needed. The Case Manager ensures a high level of personalized and sensitive support while adhering to policies and procedures, and effectively establishes supportive relationships with clients, staff, and external entities.

Key Responsibilities:

Case Manager Duties:

  • Client Engagement

  • Ensure clients and their families receive excellent customer service and can navigate their care successfully.

  • Review agency consent forms and attendance agreements with caregivers and clients, fostering informed decision-making.

  • Verify Medi-Cal eligibility and assist clients in enrolling in benefits.

  • Meet with caregivers and clients to assign cases and schedule the first treatment appointment.

  • Contact clients/caregivers within 24 hours of a missed appointment to support consistent attendance.

  • Provide referrals to external resources for families facing barriers to consistent treatment attendance.

  • Coordinate and facilitate services with clients, families, and community agencies.

  • Perform follow-ups and wellness checks on existing clients and new referrals.

  • Coordinate appointments and transportation services to ensure a seamless client experience.

  • Maintain current knowledge of agency activities, services, and community resources.

  • Documentation and Compliance:

  • Utilize concurrent documentation to prepare notes and bills for engagement interventions following DMH standards.

  • Complete necessary documentation promptly to support allowable billing of outreach activities.

  • Document direct service with clients and/or school staff in Exym, adhering to DMH standards and guidelines.

  • Support and Advocacy:

  • Provide support, advocacy, role modeling, parenting skills, and referrals.

  • Conduct individual and family basic needs assessments.

  • Establish and maintain professional and positive relationships with staff, clients, and community providers.

  • Develop intervention efforts in collaboration with therapists.

  • Participate in community outreach and promote Counseling4Kids services.

  • Engage clients in re-framing problems, focusing on solutions, and supporting progress towards goals.

  • Educate clients on coping skills, anger management, positive communication, and self-efficacy.

  • Community Involvement:

  • Actively engage and educate the public at community events.

  • Participate in community trainings to enhance professional knowledge.

  • Safety and Protocol Compliance:

  • Ensure clients and/or guardians visiting the office adhere to safety protocols and agency policies.

  • Provide translation and/or interpretation services when needed.

    Requirements

Qualifications:

  • Bachelor’s degree in Social Work, Psychology, or a related field preferred.

  • Experience in customer service, case management, or related field.

  • Knowledge of Medi-Cal and other public assistance programs.

  • Strong organizational and communication skills.

  • Ability to work collaboratively with clients, families, staff, and community partners.

  • Proficiency in documentation and billing processes according to DMH standards.

  • Bilingual Spanish
