Job Duties
Monitors and provides support to patients and facilitate various recovery-oriented activities.
Facilitates or assists with adjunct groups/meetings and performs all associated duties, including documentation.
Provides support to patients during evenings and weekends, and when needed, help to de-escalate any emotional reactions patients might have.
Collects appropriate data on bio-psycho-socials.
Completes initial family contacts and provide information on family services and document appropriately and facilitates sign-up for Family Day Program and monitor family visitations.
Provides new patients with general orientation to the facility, including but not limited to, introducing new patient, initial assignments, buddy system and introduction to the facility rules and regulations.
Monitors patients during free time and their preparation for bedtimes and conduct bed and room checks.
Communicates significant events via communication log, patient charts and verbally to key staff.
Work is typically performed in a clinical environment. Accountable for satisfying all job specific obligations and complying with all organization policies and procedures. Position Details
Education
High School Diploma or Equivalent (GED)- (Required), Bachelor's Degree-Social Sciences (Preferred)
Experience
Minimum of 1 year-Related work experience (Preferred)
